A stroke happens when blood supply to parts of the brain is interrupted, affecting the normal functions controlled by the brain. It’s a potentially life-threatening disease that calls for prompt first aid and treatment.

: Caused by blockage of a blood vessel to the brain which leads to insufficient blood and oxygen reaching the brain. This accounts for 74% of the strokes in Singapore.

: Caused by a rupture (tear) of a blood vessel which leads to bleeding into the brain. This accounts for 24% of the stroke cases in Singapore.

Call 995 to seek immediate medical help if you observe any of these warning signs of stroke.

Sudden drooping, weakness or numbness of the face, arm or leg, especially on one side of the body.

Abrupt loss of vision, feeling, coordination, speech or the ability to understand speech.

Sudden blurring or disruption of vision in one or both eyes.

Sudden severe headache with no known cause, followed rapidly by loss of consciousness.

Ask the care receiver to smile. Does one side of the face droop?

Ask the care receiver to raise both arms. Does one arm drift downwards?

Ask the care receiver to repeat a phrase. Does the speech sound slurred or strange?

 If you see any of the above signs, immediately call for emergency medical help.

A doctor can diagnose a stroke based on history and clinical examination, assisted by:

Brain scans such as Computerised Tomography (CT) or Magnetic Resonance Imaging (MRI) 

Other tests such as blood tests, ECGs (electrocardiograms - heart electrical tracing), ultrasound scans and angiograms (imaging of blood vessels) are also commonly performed to look for issues that may cause strokes to occur.

If the stroke is diagnosed early, thrombolysis, which is a procedure to break up blood clots that are restricting blood flow, may be able to reverse the effects of the stroke. Also, depending on the cause of the stroke, doctors will recommend different medication. E.g. If the stroke has been caused by a blood clot, blood thinners will be prescribed which may have to be taken long term.

Stroke caused by a burst blood vessel will require emergency surgery to stop bleeding. In other cases, surgery can be done slightly later to prevent the risk of another stroke. E.g. if the neck artery has been narrowed, surgery may be needed to open up the narrowed area.

 re-acquiring mobility skills such as walking, moving from chair to bed, etc.

 re-acquiring self-care skills such as bathing, dressing and feeding independently.

Encourage daily rehabilitation exercise: Regular movement is the best way for the care receiver to stimulate their brain.

Be helpful but not too helpful: The care receiver needs to do things themselves to get better.

Get tips from social workers or case managers: They can provide you with vital information about things like home modifications and insurance coverage.

Talk to an occupational therapist: They can advise on simple home modifications like installing non-slip mats and grab bars to minimise fall risks.

Keep track of side effects from medication: Keeping a daily log might help.

Look out for new stroke side effects: Consult a doctor if any new side effects emerge.

Provide emotional support: Many care receivers hit a plateau where they appear to not make progress. Persevere beyond that plateau and things may continue to improve.

Stroke: Recognising, diagnosing, treating and rehabilitating

There are 4 chambers in the heart: The right atrium on the upper right, the right ventricle below it, the left atrium on the upper left, and the left ventricle below it.

The right atrium receives deoxygenated blood from the rest of the body and pumps it into the right ventricle, which then pumps that blood to the lungs. At the lungs, it’s filled with oxygen and returned to the heart. The left atrium of the heart receives this oxygenated blood and pumps it to the left ventricle, which then pumps it to the rest of the body through an artery called the aorta. Arteries are the blood vessels that pump blood to various parts of the body and the aorta is the largest artery in the body.

To perform such a crucial task every moment of every day of our lives, the heart itself needs to be supplied with sufficient oxygen. This happens through the coronary arteries which branch out from the aorta and return to the heart to supply the heart muscles with oxygenated blood.

When the coronary arteries become blocked due to a build-up of plaque due to fat, cholesterol and other materials, this is called coronary heart disease. The heart muscles receive less oxygen and find it harder to pump blood. This is the most common cause of a heart attack.

What’s the difference between heart attack and sudden cardiac arrest?

The terms “heart attack” and “sudden cardiac arrest” are often used interchangeably but that doesn’t mean they are the same. A heart attack is caused by a blockage of the coronary arteries whereas sudden cardiac arrest is caused by an electrical malfunction in the heart that results in an irregular heartbeat (arrhythmia). A heart attack sufferer is usually conscious whereas sudden cardiac arrest is characterised by an abrupt loss of heart function, breathing and consciousness. A heart attack sometimes leads to sudden cardiac arrest.

What to do when someone is suffering a heart attack?

If you suspect someone of having a heart attack, time is of the essence. Responding quickly is critical because the longer appropriate treatment is delayed, the more heart muscle will die because of lack of oxygen, and the more likely the person may progress to a sudden cardiac arrest and death, or end up with heart failure.

Get to the hospital as quickly as possible. Call 995. If you can’t get an ambulance to come, have a neighbor or friend drive you.

Take an aspirin while waiting for emergency help unless you have been told otherwise before or have problems with bleeding or allergy. Aspirin helps keep blood from clotting and when taken during a heart attack, may reduce the degree of heart damage.

Take nitroglycerin as directed if prescribed before.

Start CPR if the person is unconscious, and you don’t detect a pulse or breathing (cardiac arrest). Please check out the full instructions on how to perform CPR at 

Use an automated external defibrillator (AED) if available, when sudden cardiac arrest is suspected in an unconscious person. The device will give instructions on how to use it.

These steps can keep the care receiver alive until an ambulance arrives and transports them to a hospital for professional treatment.

Shingles is an infection caused by the varicella-zoster virus, which also causes chicken pox. After contracting chickenpox, the virus stays in the body and can eventually reactivate later in life. Those who have had chickenpox before may therefore be at risk of shingles down the road, especially past the age of 50.

Manifests as a painful, itchy rash, often on one side of the body or face

Starts as small red dots that develop into fluid-filled blisters, which usually scab over within a week.

In most cases, shingles clears up naturally in 2-4 weeks

Complications arising from shingles include pneumonia, encephalitis (brain inflammation) and hearing loss

The most common complication is long-term nerve pain

While chickenpox is highly contagious - spreading via blisters, saliva or mucus of those infected - shingles is not passed from person to person. However, a person who has never had chickenpox can contract chickenpox from someone who has shingles. 

Common symptoms for the two include fever and headaches. Chickenpox usually presents with bumps that turn into fluid-filled blisters, which eventually scab and fall off. In contrast, a shingles rash presents with red patches and painful blisters, and usually affects one side of the body in a band or strip-like pattern corresponding to the skin nerve that the virus is affecting.

A healthy lifestyle, with regular exercise and good nutrition, will strengthen the immune system and prevent one from contracting shingles.

Antivirals should be started as soon as possible, and doing so within 48 hours of the symptoms appearing would be best. This can help to reduce the severity of symptoms, especially their accompanying pain. Painkillers may also help to relieve some of the pain. Other treatments may include creams or lotions to help relieve itching, cool compresses, steroids and, in some cases, antidepressants and anticonvulsants.

For adults, shingles vaccinations are highly recommended for those aged 50 and above, as well as individuals with diabetes or weakened immune systems. Children should also be vaccinated against chickenpox.

If you suspect that you or a loved one has shingles, do consult a medical professional early for a proper diagnosis and treatment plan, as early intervention can minimise symptoms and prevent complications.

2. “You should give in to them/give them what they want”

Sometimes, relatives might suggest giving the care receiver what they ask for, even if it’s not in their best interest. This can be harmful, especially if the care receiver has dietary restrictions or medical conditions. For example, a care receiver may want a sugary drink that could worsen their health.

What to say: “I need to balance what they want with what’s best for their health. While one drink might not seem like much, it could put unnecessary strain on their body, and I’d rather keep them safe.”

3. “Why don’t you just help them do it?”

This comment might come from a place of wanting to simplify the process, but it overlooks the importance of encouraging the care receiver's independence. While it may seem simpler to assist with tasks, caregivers often aim to strike a balance between support and autonomy.

What to say: “I encourage them to do as much as they can for their own well-being, whether it’s for their physical or cognitive stimulation. I help when they need it, but encouraging their independence is better for them in the long run.”

4. “You used to be able to do this before you became a caregiver”

When someone becomes a caregiver, their role changes, and so do their physical and emotional capacities. It’s common for new caregivers to struggle with maintaining the same standards of cleanliness or efficiency they once had.

What to say: “Becoming a caregiver has changed my routine and energy levels. I sleep much less now, for example. It's a big adjustment, and I’m doing my best to manage it.”

5. “If you think this is hard, wait till you have a child/It’s like having a child”

Remember, self-care is essential for caregivers. Set aside daily "me time" to recharge and ensure you're not constantly exhausted. 

When someone offers assistance, don’t hesitate to accept it. It’s not a weakness, but a smart way to lighten your load. Keep a list of small tasks, like errands, that can be easily delegated. 

If you're feeling overwhelmed, don’t be afraid to share your struggles with trusted loved ones. For instance, you might say, "I’ve been really tired lately, could you help me by driving my grandmother to her medical checkup tomorrow so I can rest?"

Allow others to help in their own way. People are more likely to offer support when they feel trusted and valued, rather than pressured to follow rigid instructions. 

While caregiver burnout can feel overwhelming, there are ways to manage and overcome it. Through recognizing your emotions, practicing self-compassion, seeking support, and being realistic about your capacity, you can protect your well-being. Embracing these strategies will help you avoid burnout and ensure you can continue providing the best care for your loved ones.
